Diesel hard to cold start

I have a Case 1070. When the engine is cold I have to use starting fluid to get it to crank. Cold is a relative term as it does this on a hot summer morning also. The exhaust also smokes like crazy while warming up. After the engine is at operating temperature, it stops smoking and will start easily with no problem. I'm sure it's not burning oil though. Is it time for (shudder) an injector pump rebuild? Thanks for any advice!
